If you have called our hospital and have received authorization to drop your pet off for treatment or diagnostics, please print and fill out this form [118kb .pdf] prior to bringing your pet in for the day. Also, if your patient is diabetic, please print and fill out our diabetic drop off form [129kb .pdf] instead so that we may care for your pet more appropriately.
Consent for care form
If you will be leaving town for any length of time and your pet needs to be seen due to an emergency, we will need permission to have a caretaker bring in your pet. You can give us your contact and preferred payment information, as well as caretaker information and other special instructions. Please print and fill out this form [111kb .pdf] and drop it off to be filed in your chart.
